Please note that if you are using a laptop or cell phone, you can download the PDFfiller app or continue in a browser. Provide your name and email address and click the ‘Get Started’ button. You will have to accept the electronic terms and conditions before proceeding. Please make sure that you complete all mandatory questions. Once completed please make sure you click ‘Finish’ so ensure we receive your paperwork.

After making an appointment, be sure to have the following items with you when you arrive:
1 – Picture ID (Drivers License or Passport)
2 – Insurance card(s) – This includes primary and secondary health insurance and auto insurance if related to a motor vehicle accident
3 – Medical records from any physicians that have treated you for this reason. You may email those to frontdesk@expertcarecenter.com
4 – If this appointment is related to a personal injury case, please email your documents to info@expertcarecenter.com
5 – Radiology studies, such as MRIs, CT scans, X-rays, NCS/EMV). Please note that if you had any of these tests completed at the following centers, we are able to access them online: Advanced Diagnostic, Beaches MRI, Rayus Radiology (previously Diagnostic Centers of America), Delray MRI/CT, MRI Specialists, Stand UP, Windsor Imaging, University MRI, Akumin MRI.
